It could be a reference to bruising, but even in that case, ruptures of vessels in the abd wall can cause very significant blood loss due to dissection of tissue planes by the blood. It could be internal bleeding as well which you refuse to entertain. But you falsely assume because he stood around afterwards for a short while on film that he was not hit significantly.
But the issue isn't really what he suffered, it's that in either case, it represented enough of a threat to justify his defense reaction i n that split second when this crazy protestor lost her head and gunned her engine with him standing right in front of her.
