What everyone forgets, and Trump too, is that doctors aren't doing their own billing. The billing is now done almost completely by hospitals (a phenomenon pushed strongly through regulation and penalties for not converting to EMR billing and patient care by the Obama administration). The doctors enter the information into an EMR, and then offsite billers/coders who are paid and taught how to to "maximize billing opportunities".
It should be noted that most doctors and PA's and NP's get a productivity reward based on their billings, so their certainly can be those who are not being on the up and up about it. But is is primarily the hospitals who are the culprits here. While most doctors are honest from my experience, very, very few hospitals are.
Thus, my only issue as a doctor with the isolated quote you posted is that he should blame the hospitals. but I applaud him for calling it out.
