The equipment in their army is junk or worse... liike everythiung they manufacture, especially involving anything high tech.
I think Putin has found this out over the past few months and would never dare push any button for a first strike.
If your argument is that we shouldn't worry because maybe only a fraction of the Russian hydrogen bombs aimed at this country will actually work, then you have weird priorities.
They would all work. Or almost all. It is tech from the 1950s. And there would be 1600 of them coming our way.
Of course if he is dying he may do so anyway... in which case we can only hope that his order would not be obeyed.

It seems more likely that a nuke could be smuggled into the United States across the southern border, and detonated in Dallas or Chicago, or maybe detonated on a boat in a harbor near DC or LA. Or, maybe an EMP strike like in the book, One Second After (launched from a container ship off the coast which is then sunk immediately after launch).
And what if a limited strike like one of those above occurs? Does the United States launch a full scale nuclear attack on Russia and China?...triggering them to launch a full strike against the United States? Maybe. But probably not.
The most deadly is a full exchange. But there are more likely scenarios that are survivable with the proper knowledge and preparation. And, focusing only on the most deadly but least likely scenario does a disservice to the American people. There are ways to survive. But, this analysis requires nuance.
No terrorist group could set off a nuclear weapon, thank goodness. They contain what are essentially self-destruct mechanisms that prevent unauthorized use. Plus they would be detected coming across the border.
And EMP strikes sound terrifying...but they are not realistic either. The article below is a good one.

The article you link also assumes that the United States would nuke Russia and China if Iran EMP'd the US (as in the One Second After scenario). That seems a bit unrealistic, don't you think?
But we would detect a bomb coming in through our ports or across the Rio Grande.
How would we detect something like that crossing the Rio Grande? Tens of millions have crossed our border, many not caught. We have Pakistanis crossing the border into Texas.
And they couldn’t be detonated.
