So I'll just assume that is a parody. Because a nuclear war would not be survivable for 99% of this country.
We will not all die on the first day. A large percentage of the population would survive the initial exchange.
With no government and no services (like national trucking) most would die off in the first year, first because of no medical supplies, and later, the "healthy" would die from starvation.
Now, given that, the government may decide it is not worth saving city dwellers, since they will be a drag on those who can feed themselves.
Which, to my knowledge, has not been as roundly debunked as that guy claims. Enough explosions could well alter the climate.
It is true that millions would survive that first day. The number would depend on how many weapons we were hit with. But since almost all Americans would be dead in 6 months (clean water would be hard to come by), I don't know what we are even talking about here.

Blasts won't kill everyone.
Fallout can be managed if you know what you are doing and you took precautionary steps. (Very few will do this.)
Then it comes down to food and water, and the lack of that will kill most people. I wonder if those with wells on their property would be the only safe ones? I would think the the ground would filter out the radiation particles in the well water.
