Is this yet another fashion statement by the players to wear "cut offs" and have a mouth piece dangling from their facemask? Section d says that helmets are required. Can a player stop wearing his helmet since rules are not getting enforced? Just thinking out loud.

In football there is no actual evidence however it is believed it helps with likely hood of damaging sub-concussions.
Children, high school, college students should most definitely use their mouth guards/pieces as their bodies and brains are still developing and sensitive to permanent injury.
But no - there is no evidence which shows a decline in CTE type injury.
But the evidence in football and mouthpieces is under debate, you are correct.
