A horrible accident, apparently. Airliners from Iran take a beating when the US and Iran are clashing; 290 died with the USS Vincennes accidentally shot down an airliner in 1988.

The series of unfortunate events leading up to the missile being fired are hard to buy as coincidence. I guess you never know in the fog of war though.
The Vincennes was not where it should have been, and its commander - who we gave a FRICKING MEDAL to - was a complete idiot.
But there is no way that he meant to shoot down an airliner and kill 290 people.
KAL 007 in '83, and another in '78. Seems like the fighter jets could have identified those planes before firing upon them.
I know dogfights aren't really dogfights with stand-off weapons, so pilots often don't see the plane they are shooting down. But, these were not war zones, and they took place a long time ago. Normal protocol was to put eyes on the plane to attempt visual communication.
The Soviets were on edge because the Reagan Admin had been provoking them for months - flying into their airspace and simulating attacks. They thought it was another provocation.
They did not mean to shoot down an airliner.
